Failure Cost: Why Rework, Write-Offs, and Recovery Time Don’t Show Up on Your P&L
You quoted 200 hours on a project. Your team delivered in 270. The timesheet says the project ran long. What the timesheet doesn't say is that 40 of those hours were spent fixing work that broke at a handoff nobody was watching.
Failure cost is the third component of the Cost-Per-Workflow framework — after direct labor cost and coordination cost. It includes every hour spent on rework, every dollar written off in client disputes, every recovery meeting called because a deliverable came back wrong, and every re-scoping conversation triggered by a missed requirement. In services firms between 30 and 100 people, failure cost runs 10-25% of total workflow cost, and almost none of it appears on a financial statement.
The reason: failure cost gets absorbed into project hours. The PM logs 270 hours against a 200-hour estimate and marks it as a project that “ran long.” The 40 hours of rework aren’t tagged as rework. They look like regular work on the timesheet because the timesheet doesn’t have a category for “work we did twice.”
The three components of failure cost
Failure cost breaks into three measurable categories. Each one has a different cause, a different magnitude, and a different fix.

In a 60-person services firm running 40-50 projects per year at an average project size of $80,000-$150,000, these three components add up to $180,000-$500,000 annually. The wide range reflects how much variation exists between firms with structured handoffs and firms without them.
Where failure cost concentrates
Failure cost doesn’t distribute evenly across a workflow. Cendia’s finding across 20+ engagements: roughly 80% of failure cost traces back to 2-3 specific handoff points in any given workflow. The handoffs with the highest rework rates are where the money concentrates.
The pattern is consistent. Failure cost clusters at three types of handoff:
Handoffs where context changes format. When a project moves from a verbal agreement to a written scope, from a design file to a development spec, or from a completed deliverable to an invoice. Each format change requires someone to translate context from one medium to another. Each translation introduces the possibility that something gets lost, simplified, or misunderstood. A verbal agreement that becomes a written scope with different boundaries produces rework when the team builds to the written scope and the client expected the verbal one.
Handoffs where ownership is ambiguous. When a project moves between phases and nobody is explicitly assigned to the receiving end. The project sits in a gap for 2-5 days. When someone picks it up, they lack context on what decisions were already made, what the client was told, and what the timeline assumes. The rework comes 2-3 weeks later when the downstream work doesn’t match the upstream commitments.
Handoffs where the decision rule is undocumented. When a project hits an edge case — the client requests a change, the deliverable doesn’t match the spec, the timeline is at risk — and no documented rule exists for how to handle it. The team makes a judgment call. Sometimes the judgment is right. When it’s wrong, the rework cost is 4-12 hours per event, plus the recovery meetings and client management that follow.
The Handoff Cost Model quantifies this precisely. Boundary handoffs (between teams) produce 4x the failure cost of internal handoffs (within teams) because the context translation is heavier and the ownership gaps are wider.
Why the P&L doesn’t show it
Failure cost is invisible on financial statements for three reasons:
1. Time tracking doesn’t separate rework from original work. When a developer spends 6 hours redoing a feature because the spec was incomplete, those 6 hours appear as project labor — indistinguishable from the 6 hours spent building it correctly the first time. The P&L shows total labor cost. It doesn’t show that 15-20% of that labor was rework.
2. Write-offs appear as revenue adjustments, not operational failures. When a firm gives a client a $12,000 credit because the deliverable was late and the client threatened to withhold payment, that credit appears as a revenue line item. It doesn’t appear in operations reporting as a failure cost traceable to a specific handoff breakdown.
3. Recovery time is logged as management overhead. The 4 hours the PM spent in meetings investigating what went wrong, the 2 hours the account manager spent managing the client relationship, the 1 hour the operations director spent re-scoping the project — all of this appears as general management time. Nobody tags it as “time spent recovering from a workflow failure at the design-to-development handoff.”
The result: a firm can have $300,000 per year in failure cost and see it nowhere in their financial reporting. It’s distributed across project labor, revenue adjustments, and management overhead — three buckets that nobody cross-references.
How to measure your own failure cost
You can estimate failure cost for your most common project type in about 3 hours. The measurement has two parts.
Part 1: The project gap analysis. Pull 5 recently completed projects. For each one, compare estimated hours to actual hours. The gap — actual minus estimated — is your combined coordination and failure cost. Most firms find the gap runs 25-45% of the original estimate.
To separate failure cost from coordination cost, interview the PM on each project. Ask two questions:
- “Where did the extra hours go?” (The PM usually knows — they lived through it.)
- “How many of those hours were spent redoing work that was already done?”
The rework hours are your failure cost. The remaining gap hours are coordination cost (meetings, handoffs, status updates, chase-downs).
Part 2: The write-off audit. Pull 12 months of client credits, discounts given after project start, disputed invoices, and partial write-offs. Sum them. Divide by total revenue. Most services firms in the 30-100 person range find write-offs running 2-5% of annual revenue. At $8M in revenue, that’s $160,000-$400,000 per year in work performed but never fully collected.
Part 3: The handoff trace. For each rework event identified in Part 1, trace it back to the handoff where the failure originated. “The client rejected the final design because it didn’t match the approved concept” traces back to the concept-to-design handoff. “The invoice was disputed because the client didn’t agree to the change order” traces back to the scope-change-to-documentation handoff. Map 10 rework events and you’ll find that 7-8 of them originate at 2-3 handoff points.
Those 2-3 handoff points are where 80% of your failure cost lives. Fix those and you fix most of the problem.
What actually reduces failure cost
Three interventions, in order of typical impact:
1. Documented handoff specs at the top 3 failure points. For each high-failure handoff, create a one-page spec that defines: what the sending team must deliver, what format it must be in, what the receiving team needs to confirm before accepting, and what happens when the handoff doesn’t meet spec. The Eliminate-Before-Automate framework applies — some handoffs should be eliminated entirely rather than documented. 2. A rework tag in time tracking. Add one category to your time tracking: “rework.” When a team member spends time redoing work that was already done, they log it as rework. This single measurement makes failure cost visible for the first time. Most firms that implement rework tagging discover their actual failure cost is 30-50% higher than the PM interview estimates suggested — because the PMs underestimate how much rework happens at the individual contributor level.
3. A 15-minute post-project review for any project that exceeded estimate by more than 20%. One question: “What would we do differently?” Capture the answer. The review isn’t a blame exercise — it’s a handoff improvement exercise. Three months of post-project reviews produce a running list of handoff failures that tells you exactly where your process is breaking and how often.
The timesheet says 270 hours. The estimate said 200. The 70-hour gap contains rework, recovery, and re-scoping that nobody tagged as failure cost — because nobody asked where the hours went.
What this isn’t
Scope notes:
- This isn’t a quality control argument. Failure cost isn’t about bad work. Rework happens when good people receive incomplete specs, ambiguous handoffs, or undocumented decision rules. The failure is structural — it lives in the transition between steps, not inside any one step.
- This isn’t a time-tracking pitch. You don’t need elaborate time tracking to measure failure cost. The 5-project gap analysis and PM interview method described above takes 3 hours and produces a usable number. Perfect data isn’t required — a directionally accurate estimate is enough to identify the top 2-3 handoff failures.
- This isn’t specific to any project type. Digital projects, consulting engagements, legal matters, accounting audits — failure cost follows the same pattern in any information-heavy services workflow. The handoff points differ by industry. The concentration pattern (80% at 2-3 points) holds universally.
Where to start this week
Two actions, both completable before Friday:

Pull your 3 most recent completed projects. Compare estimated hours to actual hours. Calculate the gap as a percentage. If it’s above 25%, failure cost is significant enough to investigate. Ask the PM on each: “How many hours were rework?”

Trace one rework event. Pick the most expensive rework from those 3 projects. Follow it back to the handoff where the failure originated. What was missing — a spec, an ownership assignment, a decision rule, a format translation? That handoff is your highest-impact fix.
